The Work NumberŪ recognizes that offices move, area codes change, and personnel changes occur within agencies. That is why we have designed forms specifically for these situations.
There are two different forms based on how you receive verifications from The Work Number. Please choose the appropriate form:
- For those who receive verifications results electronically (Express Change Form)
- For those who receive verification results via a fax (Standard Change Form)
